In this case report, we present an individual with primary amyloid myelopathy who presented with an unusual clinical scenario.

The individual's initial symptoms included severe weakness in the upper extremities, along and urinary incontinence. Cerebrospinal fluid analysis revealed elevated protein levels consistent with Journal of Medical Case Reports amyloid deposits. Neuroimaging studies demonstrated characteristic lesions in the spinal cord consistent with primary amyloid myelopathy.

The patient was treated supportive care, including physical therapy and medications to manage symptoms. However treatment, the patient's condition progressed steadily. This case emphasizes the importance of considering primary amyloid myelopathy in the differential diagnosis of patients presenting with progressive neurological symptoms, particularly those with asymmetric limb involvement.

A high-resolution computed tomography (HRCT) scan of the chest confirmed the diagnosis, revealing honeycombing/fibrotic bands/reticular opacities.

The patient underwent further tests/evaluation/assessments to rule out other potential causes/contributing factors/underlying conditions of pulmonary fibrosis.
